- Your contact information.
- Identification of the copyrighted work.
- Identification of the allegedly infringing material.
- A statement of good faith belief.
- A statement under penalty of perjury that the information is accurate.
- Your physical or electronic signature.

What Happens Next
Upon receipt of a valid notice, Triply may remove or disable access to the content and notify the affected user. Users who repeatedly infringe copyrights may have their accounts terminated.
